  RMA: End User  
  To obtain repair services for products under warranty, please fill out an RMA (Return Material Authorization) Form and provide the following REQUIRED information:

1. A copy of the orginal invoice for unit(s) to be repaired or replaced
2. Company Name (if applicable)
3. User Name
4. Mailing Address
5. Detailed description of problem
6. Serial Number and Manufacturing Date
  RMA Number will NOT be issued WITHOUT Serial Number
Please attach the RMA label outside of each return packages. The returned merchandise without a valid RMA Number will be rejected or return as is by ADVAN.
